The forms below are for current students only:

Academic Admin Request: The student must submit the completed form to the Academic Administration Office Secretary before 4:00 p.m. on Thursdays for it to be processed for the meeting, which usually meets on Mondays.

Change of Grade: This form facilitates a change of grade for a valid reason. The lecturer submits the form to the Registrar’s Office.

Intensive Registration: Note: Registration for a tutorial requires the following information and signatures. The student submits the form to the Records Office and obtains the confirmation slip shown below.

Transfer Credit: Transfer credits are granted to equivalent degree courses completed from accredited higher educational institutions. All transfer credits must be applied for in the first semester of joining Helderberg College.

Change of Programme: To change the programme of study requires the following information and approval. The student submits the form to the Records Office and obtains the confirmation slip shown below.

Complete Withdrawal:  To withdraw from all the courses registered in the semester requires the following information and signatures.

Credit By Examination: To obtain credit by examination for a course requires the following information and approval. The challenge by examination (CBE) for any course must be completed in the first year of study at Helderberg College.

Submission of Grade CBE, Agrett, intensive: This form facilitates submission of grade for agroetat exam, challenge by exam or tutorial. The form requires the following information and signatures. The lecturer submits the form to the Records Office.

Supplementary Grade: This form facilitates submission of supplementary grade(s). The form requires the following information and signatures. The lecturer submits the form to the Records Office.
